Independent mental health homicide report into the treatment of Valdo Calocane

On 5th February 2025 NHS England (Midlands) published the full independent investigation conducted by Theemis into the care and treatment of Valdo Calocane in the months leading up to the events of 13 June 2023. Valdo Calocane was convicted of the killings of Ian Coates, Grace O’Malley-Kumar and Barnaby Webber in January 2024.

The NHS has taken the decision to publish the report in full in line with the wishes of the families and given the level of detail already in the public domain. The aim of this investigation was to thoroughly review the NHS care and treatment provided to Valdo Calocane by Nottinghamshire Healthcare NHS Foundation Trust prior to the tragic events of 13 June 2023, and the interactions the NHS had with other agencies involved in his care.

Some of the failings identified relate to the involvement of his family in his care. The report states the voice of Mr Calocane’s family “was not effectively considered to support the dynamic evaluation of risk” during his treatment. Mr Calocane’s mother Celeste and brother Elias say there were a series of missed opportunities over three years to prevent the tragedy.

The report recommends developing strategies to actively engage families, carers and support networks and the co production of care plans. The report also recommends working in line with national guidelines including the ‘Triangle of Care’ and the ‘Patient and Carer Race Equality Framework‘.
